BUILDING YOUR PORTFOLIO OF EVIDENCE AND REFLECTIVE STATEMENT This guidance will help you build the Portfolio of Evidence needed for your end-point. assessment. It will help you work out what to put in your Portfolio and how to organise the evidence. The guidance will also help you organise your Training Log, and outline what is required for your Reflective Statement, which must be included.
There is no set form for a witness statement, however, you should write your statement in a clear and logical way. You should also write the statement using your own words. When setting out your statement is a good idea to: use white A4 paper; type if possible, or use neat, clear handwriting in black or blue pen; use only one side of the paper.
Witness statement template is a summary of oral evidence that a witness will give at trial. Generally the statement start with the name of case, claim number and also state the full name, address of the witness. It should record what the witness saw, heard or felt and it is also important to record anything that may open up new line of inquiry in a case. The statement must intend to be signed.
An NVQ should be candidate-led ie the candidate dictates the pace and structure of the award with agreement with their assessors. Evidence can be gathered in a number of different ways - by direct observation - the most important way as this is how you are judged to be competent; you can write self-reflective reports about your day to day practice in the workplace; you can get other people to.
A reflective account is usually a write up of how you have carried out part of your job in the past. It could be yesterday or a week, month or years ago as long as you use past tense. It maybe how you dealt with a health emergency, an abusive situation or how you have used a particular piece of equipment in the past. Your account will be a record of events that actually happened. It must be.
As previously mentioned, a great way to support the QCF candidate is to write evidence about their role to show how they are meeting specific criteria. Although the candidate will be using a wide range of evidence to complete their QCF qualification the best three forms of evidence for mentors to use are: - Direct observation - Witness testimony.
